Charlestown Ambulance-Rescue Service is working with a graduate student and Dr. Michael Hall at Roger Williams University. We are conducting a program evaluation that seeks to evaluate patient satisfaction with the Charlestown Ambulance-Rescue Service.

We're asking if you are willing to take 10-15 minutes of your time to complete a short survey. Participating in the survey is completely voluntary and the data provided will be kept confidential. Your participation may assist the ambulance service when making management decisions about their program in the future.
